I upgraded to 9.1.2.1, Not sure if I did it right since I couldn't find upgrade instructions anywhere. I just decompressed locally and uploaded everything. (I did do a full backup first.)

The basic site index.php shows Maintenance and so does the admin.php. I can't login to change anything. I cleared the site cache, I set the cms_config_custom to maintenance=0 and I even made sure the Default_Theme=default. What can I do to fix this?

As you do with all installs/upgrades, run install.php.
