New block manager's nice, but on my system it's not saving any changes made in the JavaScript area (add, delete, order changes).

Yup. I even looked at the video tutorial to see if I was doing something wrong. When I press update, the page reloads (which I'd expect), but nothing is saved.
I don't know if it's relevant, but I get these warnings:

try to clean your cache, try to upload all files including cmsinit.inc.

It looks like a module without blocks, but the script is aware of those. Till now i cannot reproduce it. Do you mind to zip and attach your cacke/config_blocks_list.php please.
[edit]Yes got it ... its a relic block assigned to a not existing module.[/edit]
[edit]The upgrade will now skip relic blocks still assigned to uninstalled modules, probably an old bug.[/edit]

Command executes, but doesn't solve the problem for me. After arranging the blocks and pressing "Update Changes", the blocks shuffle back to the order they were in.
Look like I found a legitimate bug, at least.

did the query affected any rows?
do you still see the exact same warnings as before or after the query some of them disappeared.
also if you comment line 76 in admin/modules/blocks.php where the url_redirect is then you should be able to see any possible warning.

This is puzzling me so much thats its unbelievable, others users upgraded to 9.1.2.6 and the blocks are working perfectly and its even working on my old PostgreSQL installation upgrade and new install with no one single issues.

Yeah I've fixed few bugs already reported on our bug tracking plus fixed the favicon url which was filling up our error log and applied a patch for the above bug coming from relict data.

Same issue as Grue here with the Line 181 undefined offset spam when debugging is on. I tried that SQL statement you posted for Grue, and got zero matches (no changes).
Also, have an additional problem in that I cannot drag and drop any of the blocks in the blocks admin (right hand side). When the mouse hovers over them I just get the circle with the line through it cursor.
